Our professional staff is dedicated to your health, safety and well-being as you, a patient or loved one recovers from surgery, an acute illness or is managing a chronic illness at home. We work directly with individuals, as well as hospital discharge planners and other medical professionals throughout Dane County. Following a referral or individual call, our Registered Nurse Medicare Specialist conducts a comprehensive assessment to determine and submit the order for skilled service/therapy. Our staff included Registered Nurses, Certified Nursing Assistants, Physical, Occupational and Speech Therapists.

We now provide home health care services to patients who have private insurance plans and/or Medicare.
We have a comprehensive list of services including the following:
Exercise to improve joint and muscle function following surgery, a fall or increased risk of falling
Monthly catheter changes
Patient and family education
Assessment of swallowing problems
